Voluntary Vision Coverage through Anthem's BlueView Vision Plan is available to full time employees and their eligible dependents. Coverage under this plan allows those enrolled to seek vision care from in-network providers with low copayments, and provides discounts on lenses and frames.

| Standard provides regular, full-time employees with a Group Term Life insurance equal to four times their annual salary. This company-paid group life insurance is provided through Prudential. In addition to the company-provided life insurance, we offer supplemental insurance to increase coverage to up to four times your annual salary. |

| All full-time employees are eligible to participate in Flexible Spending Accounts for Medical and/or Dependent Care expenses. Flexible Spending Accounts allow employees to pay for non-reimbursed medical and daycare expenses with pre-tax dollars. |

| Short Term Disability insurance is provided to full-time employees and is 99% Employer Paid after the eligibility period is satisfied. Long Term Disability is provided at no cost to employees. Eligibility begins after a 90-day waiting period, and pays 60% of the employee’s annual salary. |

| Standard offers a 401(k) savings plan including a company contribution to assist employees in saving for retirement and attaining financial goals. All employees are eligible to participate on the first day of a new quarter following 30 days of employment. Contributions may be made up to 90% of earnings to the Plan on a pre-tax basis (up to the maximum allowed by the Internal Revenue Code), provided taxes and deductions are covered. There are a diverse selection of funds in which to invest. |
